COVID-19 Screening for Candidates
To protect everyone, we are asking all candidates to complete the following questionnaire.
Have you suffered from any potential Coronavirus symptoms in the last 14 days? (These
include a
temperature, respiratory symptoms, cough and/or shortness of breath)
Has anyone in your house suffered from any potential Coronavirus symptoms in the last 14
days?
(These include a temperature, respiratory symptoms, cough and/or shortness of breath).
Have you suffered from any other cold or flu like symptoms in the last 14 days?
Have you returned from an effected country in the last 14 days? (China, Iran, Korea,
Italy,Japan,Laos,Macau,Malaysia, Myanmar, Singapore, Taiwan, Thailand, Vietnam)
Please list countries you have visited in the last 30 days.
Have you had any known contact with a confirmed case of Coronavirus?
Please see below the hygiene measures you need to take when you are on site.

Cover your mouth and nose with a tissue or your sleeve (not your hands) when you cough or
sneeze

Put used tissues in the bin immediately

Wash your hands with soap and water often – do this for at least 20 seconds

Try to avoid close contact with people who are unwell

Don’t touch your eyes, nose or mouth if your hands are not clean

Ensure where possible to keep a distance with others of 2 metres

If you become unwell whilst at work or start to feel any of the symptoms quoted above you
must
contact a supervisor and your agency straight away and organise to go home and self-isolate
